Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. We are also able to utilize most HSA& FSA cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
We work for our patients, not the insurance companies. We are committed to maximizing any dental benefits you may have but because of the intricacy of insurance, we can only estimate your coverage in good faith, to the best of our abilities. We are not currently contracted with any HMO or medi-cal/medi-care insurance policies, at this time. Please contact our office for more insurance details, we look forward to hearing from you!
